Effective March 18, 2018 and pursuant to the amended Civil Service Law Section 159-b, Wilson Central School District employees are entitled to excused leave for up to four (4) hours per calendar year to
undertake a screening for cancer of any kind. See WCSD Policy #6550.

Take this Cancer Screening Leave Form to your doctor's appointment (it is acceptable for the office staff to stamp it). Submit the completed and signed form to  your building attendance person.

The US Department of Labor requires employers to designate leave as FMLA qualifying once enough information is received to determine if it qualifies. 

During FMLA leave, health and dental insurance benefits, if any, will be maintained under the same conditions as if you continued to work and, to the extent required by law, you will be reinstated to the same or an equivalent job with the same pay, benefits, and terms and conditions of employment upon your timely return from FMLA leave. 

FMLA can be designated for one or more of the following reasons: 

• The birth or placement of a child;
• To care for the employee’s spouse, child or parent who has a qualifying health conditions;
• For the employee’s own qualifying serious health condition that makes the employee unable to perform the emplyee’s job;
• For qualifying exigencies related to the foreign deployment of a military member who is the employee’s spouse, child or parent.

Please provide your supervisor with information that includes the expected duration of your leave, as such information becomes available.

Yes. Enter your absence into Frontline to start the process of substitute coverage. Follow the rest of your normal absence procedures. 

No. When you have IEP meetings, although you might need to enter the absence in Frontline to obtain sub coverage, you do not need to enter the absence into the portal because you are still reporting to work and not absent.

If you need to modify or delete a leave request that has already been approved by your supervisor, send an email to Sue Anderson with cc to your supervisor and include any necessary details.
